Key Responsibilities: ?
- Routinely travel to patients' homes to deliver care, educate, conduct evaluations, and offer ongoing support, ensuring personalized and effective treatment.
- Demonstrate comfort and professionalism when working in diverse patient home environments, ensuring a compassionate and safe care experience for each individual.
- Collects and records relevant patient health data in the course of/pre/post hemodialysis treatment;?
- Develops and maintains a plan of care that describes interventions to attain expected outcomes;
- Implements and records interventions to attain expected outcomes in EMR;?
- Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections;
- Perform clerical tasks in addition to patient care (i.e., filing, inventory; management, admission documentation, creating patient charts, etc.);?
- Maintains a positive and respectful attitude to all patients and colleagues; and
- Medication administration and documentation?
Requirements: ?
- Valid RN license
- Valid state driver's license
- 2 years of RN experience required
- 1 year of hemodialysis required
- 1 year of Nxstage machine required
- BLS certification; and
- Ability to lift up to 35 pounds
- This is a safety sensitive position
